Looking for work as an elderly care assistant.
• Undertake key work sessions; identify needs and risks through which support plans are developed to meet assessed needs.
• Raise customers’ awareness of local services and facilities and support engagement with these services.
• Support customers to ensure they are in receipt of all entitled benefits and services.
• Manage customer risk by ensuring close monitoring of customers’ health.
• Comply with all Group policies and procedures in delivering the service.
• Provide support in partnership with other statutory and voluntary agencies in the community, liaise with other agencies as necessary to ensure customers receive appropriate advice and support.
